Welcome!

Below is some information to help make your first visit to our office a little clearer. On the left, are the new patient forms for you to fill out and bring with you to your first visit. This will save you some time when you come in.

What you can expect at your first visit:

1) A thorough discussion of your current problem(s)

2) A thorough examination and orthopedic testing. This will help us determine which course of treatment is best for you.

3)X-rays may be taken depending on the nature and severity of your symptoms. If you have copies of previous x-rays, MRI’s or reports from these procedures, we would ask that your bring them with you to your initial visit.

4) We will lay the foundation for your treatment plan. The plan includes three phases

    Phase 1: Relief care:

We will work with you to try to provide immediate pain relief of your worst symptoms. This phase of treatment typically lasts 4-6 weeks and requires  2-3 visits a week during this time

    Phase 2: Corrective/Restorative Care:

During the corrective/restorative care phase your muscles and tissues are allowed to heal more completely. This will also to help prevent future injuries. Typically care will be 4-8 times a month for 12 to 24 months depending on the severity of your problem.

    Phase 3: Wellness Care

Continued care allows the body to heal completely, restore damaged muscle and tissues and help prevent future injuries. This will also keep other problems from arising in the future. Typically care is once every 4 to 6 weeks depending on the lifestyle and goals of the patient.

You can expect the first visit to last approximately forty-five minutes to an hour.

What you can expect at your second visit:

The doctor will go over your specific treatment plan and give you a copy of his report of findings for your records. You will also receive a copy of your x-ray report if x-rays were taken on the first visit. A home care exercise program may also be given along with any recommendations for orthotics or other supports. This visits may be between 30-40 minutes. Subsequent visits will be approximately 15-30 minutes depending on the type of care recommended for you.

Continuing Care

As your care progresses treatment will be altered as needed. Patients will see a reduction in the number of visits as they move through the three phases of care. Phase 1 is the most intensive phase and will require the most visits. During the second phase you will see a reduction in the number of visits as your spinal strength increases and your condition improves. Once you have completed this phase you will move into phase 3. Phase 3 includes a preventative approach and is to help the patient prevent the condition from returning and allows your spinal strength to continue to improve along with allowing muscle tissue to continue to heal.

This is an update form so we can quickly bring your records up to date and is the only form you need to fill out unless there is a new injury related to work or a car accident.
